Alameda County's Getting the Most Out of Life and the East Bay Conversation Project invite you to visit Conversation Village on National Healthcare Decisions Day, April 16, 2015.
This free event is open to the public, and is part of a larger movement to normalize “the conversation.” The goal of this event is to get people to consider, communicate and document their healthcare wishes through having end-of-life care conversations followed by the completion of an advance healthcare directive. The event will include a series of tents where people can choose to share their stories and experiences with end-of-life care, and also discuss their personal choices about end-of-life care with trained facilitators, coaches and champions. Conversation Village will also host vendors, music, and feature a noon time program with representatives from the City of Oakland, healthcare providers, and other community members.
Advance care planning is important regardless of age. This year, in celebration of National Healthcare Decisions Day, come to Conversation Village where you can discover health care resources, listen to stories from others in your community, and complete your own advance directive.
